2012 Spring Registration

Registration Dates:  January 3 - January 30

Registration Forms
For your convenience, all three forms are fillable pdf files.   Please type your information directly into the forms, print them, and mail all three completed forms with payment 
to the address listed below. 

Registration requires 3 completed forms and payment.  Players will not be allowed to practice or play until all 3 forms are submitted.

Late Registrations: Registrations received after January 30th will be assessed a $30 late fee and are subject to availability on the roster.  Once team rosters have been submitted to the league, no additions will be allowed.

League Communications: All league communications will be done via the DUSC website and email.  All players/families must have an active email address in order to receive important announcements regarding DUSC updates and information.

Uniforms

Team jersey, black shorts, black socks and approved soccer cleats will be required of all players.  New players will need to order a jersey on the registration form.  Players may choose their own black shorts (solid black with an inseam of at least 5 inches) and black socks.  Players will be required to provide their own soccer cleats.  A practice t-shirt will be included with the registration fee; please note the desired size on the registration form.  The practice t-shirt must be worn to all practices.  Put it over a long-sleeve t-shirt or sweatshirt if the weather is cold. Extra t-shirts may be purchased through our spring fundraiser, if desired.  

Coaches:  

Coaches will be assigned once teams are established.
